A high pressure aluminium gas cylinder ruptured violently whilst in stock, prompting an investigation to determine the root causes and remedial actions. With the help from global experts, I carried out an analysis of relevant data, a review of relevant prior incidents, and a risk assessment. The findings of the investigation and the recommendations of the risk assessment led to a new national maintenance directive for these gas cylinders.
